Please ensure compliance before forwarding these pledge forms: There is no minimum pledge amount for contributions to the Community Impact Fund, United Way’s general fund, or to Impact Areas.
All or a portion of an employee’s contribution may be directed toward one or more United Way of Greater Cleveland partner agencies or to another United Way.
The minimum directed pledge is $50 per designation and will be subject to a small processing fee.
Designations to agencies not affiliated with United Way of Greater Cleveland may not exceed four agencies per contributor, nor total more than 50 percent of a contributor’s entire annual gift.

Therefore, a minimum pledge of $100 is necessary to direct 50 percent, or $50, to a non-affiliated agency. Each non-affiliated agency must be a 501(c)(3) health and human services organization located in and serving Northeast Ohio.
United Way of Greater Cleveland does not certify the financial or program viability of these agencies that are not reviewed by our citizen review volunteers.
All designated agencies are required to comply with the USA Patriot Act and other counterterrorism laws.
In the event that an agency does not comply with these laws, funds designated to that agency will be directed to the Community Impact Fund.
Capital campaigns or cultural, political, environmental, governmental, educational and religious organizations are not eligible to receive designations.
Designations to United Way of Greater Cleveland partner agencies will be paid separately from their annual allocation. Please submit copies of all pledge forms.
If it is necessary to provide a list of employees who designated, please provide the following information for each employee listed: first and last name, home address, designated agency(ies) or Impact Area(s), designated amount(s), total gift amount and payment method.

Designation payments, less a small processing fee, are made quarterly based on collections.
Your United Way has a fiduciary responsibility to respond to donor, agency and company questions regarding these payments. All designation information must be submitted by 12/31.
